How We Verify Visits: Pavillio EVV

Minnesota law requires home care agencies to use an Electronic Visit Verification (EVV) system to confirm that scheduled visits actually happen, at the right time and place. Abriva uses Pavillio EVV for this purpose.

When a caregiver arrives for a visit, they check in using the Pavillio app, and check out when the visit ends. This confirms the date, time, and location of care.

What This Means for You

Your caregiver will use their phone to clock in and out during each visit, typically from inside or right outside your home. EVV does not track your caregiver's location outside of scheduled visit times.

Family Access

Family members or authorized representatives who want to view visit information may be able to use the Pavillio app as well. Ask our office if this applies to your care plan.

Questions About Visits?

If you ever have questions about a visit record, you or your case manager can contact our office directly at (763) 237-6351.

Your Privacy & Health Records

Abriva is committed to protecting your health information. We only use or share it to provide your treatment, obtain payment for your care, and run necessary health care operations. For more information about HIPAA and patient rights, visit the U.S. Department of Health & Human Services (opens in new tab).

Your Rights

  • To inspect, request, and receive a copy or summary of your health record.
  • To request restrictions on how your information is used or shared.
  • To request an amendment if you believe your record is incorrect or incomplete.
  • To request an accounting of certain disclosures made by Abriva.

You may express a concern about your privacy directly to Abriva or to the U.S. Department of Health and Human Services, and you will never be retaliated against for doing so.

If Something Isn't Right (Grievances)

We want to know if something about your care doesn't feel right. You're always welcome to talk with any staff member you're comfortable with, and you may ask for help putting your concern into writing at any time.

How to File a Grievance:
  1. Talk to a staff person you feel comfortable with about your complaint.
  2. Clearly tell the staff person that you are filing a formal grievance, not just an informal complaint.
  3. If not resolved, bring the complaint to the highest level of authority: the Owner at (763) 237-6351.

Response Time: Grievances affecting health and safety are responded to promptly. All other complaints are responded to within 14 calendar days and resolved within 30 calendar days.

Incident Reporting

Our staff are trained to closely observe and promptly report changes in your condition to your supervising nurse. If an incident occurs (like a fall, injury, or medication error), it must be documented immediately.

Family members will be informed when appropriate, and a detailed report will be completed and reviewed to ensure your continued safety.

Medication Safety

For your safety, Abriva caregivers follow strict medication protocols. Never share medications, and always store them securely. For information on safely disposing of unused or expired medications, visit the DEA Drug Take-Back Program (opens in new tab).

Health Care Directives

A Health Care Directive allows you to name someone to make health care decisions for you if you become unable to do so. For official Minnesota forms and information, visit the Minnesota Board on Aging (opens in new tab).
